How to reconstitute Dulaglutide
- Reconstitute lyophilized dulaglutide with bacteriostatic water (0.9% benzyl alcohol) or sterile water for injection.
- Add diluent slowly down the vial wall and roll gently to dissolve; do not shake, and discard cloudy or particulate solution.
- Store lyophilized powder at -20°C long term; keep reconstituted solution refrigerated at 2-8°C and use within 28 days.
- Marketed pens need no reconstitution: refrigerate unopened pens at 2-8°C, and discard any pen kept at room temperature past 14 days.
Frequently asked questions
Is dulaglutide FDA-approved, and what is Dulaglutide approved for?+
Yes. Injectable dulaglutide (Trulicity) is FDA-approved as a once-weekly subcutaneous injection for type 2 diabetes (T2D) in adults, and also for cardiovascular (CV) risk reduction in adults with T2D at high CV risk, alongside injectable semaglutide and liraglutide (RCT). It is not approved as a standalone obesity treatment; obesity labels in the incretin class include liraglutide, semaglutide, tirzepatide, and orforglipron. Dose-dependent weight loss is nonetheless a consistent effect in T2D trials. Completed phase 3 data in youth aged 10 to under 18 (AWARD-PEDS) support pediatric use.
What is the standard dulaglutide dose, and is there an oral form of dulaglutide?+
There is no oral dulaglutide; the drug is injectable only, given subcutaneously in the abdomen, thigh, or upper arm. Standard dosing starts at 0.75 mg once weekly, with 1.5 mg as the usual maintenance dose; escalation to 3.0 mg and then 4.5 mg weekly is available for adults who need additional glucose or weight control (RCT). Step-up in 4-week intervals minimizes gastrointestinal effects: modeling shows nausea and vomiting increases of under 3% with 4-week escalation steps. In AWARD-PEDS, 0.75 mg and 1.5 mg weekly reduced HbA1c by 0.6% and 0.9% versus a 0.6% rise on placebo at 26 weeks, with 51% reaching HbA1c below 7% versus 14% on placebo (RCT). Nausea is the most common adverse event, reported by 13-16% in AWARD-11 and 28.4% in the combined tirzepatide/dulaglutide SURPASS-CVOT exit-interview population.
What should I do if I miss a weekly dulaglutide dose?+
If a once-weekly dose is missed, inject it as soon as possible when at least 3 days remain before the next scheduled dose. If less than 3 days remain, skip the missed dose and take the next dose on the usual day, then resume the regular weekly schedule. Do not take two doses in the same week to catch up (prescribing information, standard practice).
How does dulaglutide compare with semaglutide and tirzepatide for weight loss and glucose control?+
The newer agents produce greater weight loss. Real-world 52-week data: semaglutide -6.51% versus dulaglutide -1.14% body weight, with ≥5% loss in 51.7% versus 25.0% of patients (observational). An indirect comparison found semaglutide 2.0 mg beat dulaglutide 3.0 mg and 4.5 mg by -3.29 kg and -2.57 kg body weight and -0.44% and -0.28% HbA1c (network meta-regression). A network meta-analysis ranked tirzepatide 15 mg highest for weight loss at week 16, while tirzepatide 1 mg showed the numerically smallest reduction (MD +4.39 kg versus tirzepatide 5 mg), and dulaglutide 0.75 mg showed a reduction of MD +3.10 kg. In SURPASS-CVOT, tirzepatide was non-inferior to dulaglutide 1.5 mg for MACE but lowered body weight and HbA1c more, with gastrointestinal events 42.5% versus 35.9%; the trial compared maximum tirzepatide (15 mg) with sub-maximal dulaglutide (1.5 mg) (RCT). Dulaglutide itself is not weight-neutral: AWARD-11 showed -3.0 kg on 1.5 mg and -4.6 to -4.7 kg on 4.5 mg at 36 weeks.
Can I use dulaglutide during pregnancy, or with a personal or family history of medullary thyroid cancer?+
No in both cases. Dulaglutide is contraindicated with a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia type 2, a boxed warning derived from rodent tumor data (practitioner consensus plus rodent evidence). A case report documented no calcitonin rise during 6 months of dulaglutide 2.0 mg in a patient with pre-existing MTC, but the contraindication remains in force (case report). For pregnancy, human safety has not been established; GLP-1 receptor agonists are not recommended during pregnancy or breastfeeding, and dulaglutide should be stopped well before conception because of its long half-life (practitioner consensus).
How long can I stay on dulaglutide?+
Indefinitely, as chronic therapy. The pivotal AWARD-11 trial ran 52 weeks, and the REWIND cardiovascular outcomes trial followed patients for a median of several years, yielding an NNT of 72 for MACE-3 with dulaglutide 1.5 mg (RCT). Gastrointestinal intolerance attenuates over weeks as tolerance develops, which is why dose escalation is stepped. No maximum treatment duration is defined, and glycemic durability is maintained through 52 weeks in trial data.
Does dulaglutide cause sexual dysfunction?+
Evidence points the opposite direction. A meta-analysis of 237 males found GLP-1 receptor agonist therapy improved erectile dysfunction scores (MD 2.73; 95% CI 1.64-3.83) and significantly increased total testosterone, SHBG, and FSH (meta-analysis of small studies). A FAERS disproportionality analysis found no significant erectile dysfunction reporting signal for dulaglutide, unlike semaglutide, sitagliptin, and dapagliflozin (pharmacovigilance, hypothesis-generating). Weight reduction and better glycemic control are the probable mediators.
Do I need to stop dulaglutide before surgery?+
Yes, with planned timing. The FDA added a warning to dulaglutide and the other GLP-1/GIP agents about pulmonary aspiration during general anesthesia or deep sedation, from residual gastric contents despite preoperative fasting. Standard practice is to withhold the weekly dose in the week before elective surgery, holding roughly one dosing interval (7 days), and to apply aspiration precautions in any patient with nausea, vomiting, or bloating (practitioner consensus).
Does dulaglutide need to be refrigerated, and can I travel with dulaglutide?+
Yes, refrigerate. Store unopened pens at 2-8°C (36-46°F), protected from light, and never frozen. A pen removed from refrigeration may be kept at room temperature up to 30°C (86°F) for a maximum of 14 days and then discarded (prescribing information, standard practice). For travel, carry pens in an insulated cool pouch, never in checked baggage that can freeze, and never leave them in a hot vehicle (practitioner consensus).
