How to reconstitute Exenatide
- Reconstitute the lyophilized powder with bacteriostatic water, added slowly against the vial wall.
- Roll the vial gently to dissolve; avoid vigorous shaking, which can denature the peptide.
- Store unreconstituted powder at -20°C for long-term research use; 2-8°C is acceptable near-term, with aliquots to avoid repeated freeze-thaw cycles.
- Keep reconstituted solution refrigerated at 2-8°C, protected from light, and use within 28-30 days.
Frequently asked questions
Is exenatide FDA-approved?+
Yes. Exenatide (Byetta) was the first FDA-approved GLP-1 receptor agonist, approved in 2005 for type 2 diabetes, built on exendin-4 isolated from Gila monster venom in 1992. A once-weekly microsphere formulation (Bydureon) followed. Obesity indications went to later agents such as liraglutide and semaglutide, not exenatide.
What are the standard exenatide doses?+
Start at 5 mcg subcutaneously twice daily at mealtimes for 4 weeks, then 10 mcg twice daily (RCT). Once-weekly Bydureon or Bydureon BCise: 2 mg weekly; the 2 mg autoinjector reduced HbA1c by -1.4% versus -1.0% for 10 mcg twice daily over 28 weeks (RCT). Gradual titration is evidence-based: dose escalation cut nausea from 56% to 27% (RCT). Exenatide's approved formulations are injectable only.
How much weight can I expect to lose on exenatide?+
Modest, dose-dependent loss. In 30-week placebo-controlled RCTs in type 2 diabetes, 10 mcg twice daily produced -2.8 kg (RCT). A 2026 network meta-analysis found exenatide reduced body weight by -2.97 kg versus placebo, with moderate-certainty evidence (RCT).
How does exenatide compare to semaglutide?+
Semaglutide wins decisively on weight: -10.98 kg versus -2.97 kg for exenatide in the same network meta-analysis, both moderate certainty (RCT). Glycemic effect is closer: once-weekly exenatide 2 mg reached -1.4% HbA1c. Safety differs: semaglutide carries the strongest GERD pharmacovigilance signal (ROR 3.44-6.93) while exenatide shows an inverse association (ROR 0.60); NAION signals concentrate on semaglutide, with exenatide findings absent or inconsistent.
Does exenatide cause nausea, and how do I manage Exenatide?+
Yes. Nausea is exenatide's most frequent adverse event, usually mild to moderate and transient (RCT). Escalation prevents most of it: priming from 0.02 mcg/kg three times daily, increasing 0.02 mcg/kg every 3 days, reduced nausea to 27% versus 56% in naive subjects, without loss of glucose lowering (RCT). The weekly formulation reports fewer GI events: 22.7% versus 35.6% with twice-daily dosing.
Can I use exenatide while pregnant?+
Human data are absent. In an ex vivo perfused human placenta model, exenatide crossed negligibly, with a fetal-to-maternal ratio of at most 0.017 (ex-vivo). Human teratogenicity is unstudied; clinical practice is to discontinue GLP-1 therapy before or upon confirming pregnancy (practitioner consensus).
Does exenatide work for Parkinson's disease?+
Phase 3 evidence says no. Two earlier phase II trials suggested motor benefit, but Exenatide-PD3 (n=231) showed no benefit over placebo, while a parallel trial of lixisenatide slowed motor progression (RCT). One interpretation: participants lacked the insulin desensitization that may mediate benefit (hypothesis). Current phase 3 data do not support off-label exenatide for Parkinson's.
Does exenatide need to be refrigerated?+
Yes: Byetta requires cold storage. Unopened pens should be kept at 2-8°C; a pen in use may be held at room temperature up to 30 days (practitioner consensus). The weekly Bydureon pen contains PLGA microspheres that need vigorous shaking (>15 seconds) to resuspend before injection; depot-related injection-site swelling can persist for weeks.
