New thymosin beta-4 studies show liver protection and glioma spread

Two 2026 studies looked at full-length thymosin beta-4. One found liver protection in a rat model of blocked blood flow, and another found a role in helping glioblastoma move along white matter.

A rat study tested whether full-length thymosin beta-4, a 43-amino-acid peptide, could protect the liver when blood flow is cut off and then returns 1. The peptide reduced markers of cell damage and inflammation when given before the blockage or before blood flow resumed 1. A separate study found that the same peptide, released by support cells in the brain, helped glioblastoma cells travel along white matter 2. Both studies looked at full-length thymosin beta-4, the parent peptide, not the TB-500 research summary fragment. DOI+2

What the liver study found

The liver study used 32 rats split into four groups of eight 1. One group was left uninjured, one had liver ischemia, one received thymosin beta-4 before the blood supply was blocked, and one received it before blood flow was restored 1. Ischemia means blood flow is cut off, and reperfusion is the return of blood, which can add its own damage 1. The stoppage lasted 90 minutes, and the return lasted 180 minutes. DOI

Researchers measured signs of oxidative stress, which is damage from reactive molecules, along with proteins that drive inflammation and cell death 1. The proteins included TNF-α, IL-6, and IL-1β, which call immune cells into damaged tissue 1. Both thymosin beta-4 schedules reduced these markers compared with untreated injured rats 1. Liver enzymes that rise when liver cells are hurt also fell, and tissue structure improved under the microscope 1. The limitation is that this was one rat study with eight animals per group, and it tested full-length thymosin beta-4, not TB-500. DOI+1

What the brain study found

The second study asked why glioblastoma, an aggressive brain cancer, moves through the brain's white matter 2. White matter is the wiring that connects brain regions. Researchers found that oligodendrocytes, the support cells that wrap those wires, gather at tumor edges and release thymosin beta-4 2. The study refers to this peptide as TMSB4X, the gene name for thymosin beta-4 2. That peptide switches on a growth pathway in cancer cells and helps them spread. PubMed

The cancer cells send a return signal that makes the support cells produce even more thymosin beta-4, forming a loop 2. When researchers blocked that loop in genetic and drug experiments, the tumor spread along white matter slowed in living models 2. The limitation is that this work does not test a treatment in people, and it also concerns full-length thymosin beta-4, not TB-500. PubMed+1

How this fits earlier research

Most direct evidence for TB-500 remains in animal and cell studies, not human trials. A review of 80 thymosin beta-4 and TB-500 studies found the only direct TB-500 musculoskeletal efficacy study was a rat Achilles tendon study 5. In that study, TB-500 at 60 micrograms per kilogram per day for four weeks improved tendon strength and collagen organization 6. That study also tested BPC-157, and the combination added no benefit over TB-500 alone. DOI

Two direct fragment studies sit outside muscle. One used mice bred to show Alzheimer's-like brain changes and found that thymosin beta-4-derived peptides improved maze and object-recognition performance and reduced brain inflammation 7. Another used a gel that releases TB500 and found faster healing after a chemical burn to the eye surface in mice. DOI

Human data are concentrated in full-length thymosin beta-4. A 96-patient trial tested recombinant human thymosin beta-4 after a heart attack and found no overall reduction in dead heart tissue, but a benefit in the subgroup treated within eight hours of restored blood flow 9. Late-stage eye trials have also studied the parent peptide for corneal repair 10. The new liver study adds to that parent peptide profile. It sits alongside 2026 work in sepsis-related kidney injury 11 and 2026 work in mouse heart attack 12. Most of these studies lean one way, showing that full-length thymosin beta-4 dampens inflammatory and oxidative injury in damaged tissues. DOI+2

The brain study points the other way for cancer spread. Earlier breast cancer work showed that thymosin beta-4 can promote cancer cell growth and blood vessel formation 13. The new study extends that pattern to glioblastoma 2. There is no contradiction. Tissue repair and cancer spread can share the same cell movement and growth pathways. PubMed+1

TB-500 is not the parent peptide

Full-length thymosin beta-4 is a chain of 43 amino acids, the building blocks of proteins 14. TB-500 is a short piece of that chain, the synthetic 17-23 fragment built around the LKKTETQ actin-binding region 3. Actin is a protein that helps cells move. The full-length peptide also carries the Ac-SDKP region, which has its own scar-reducing actions 4. Because TB-500 lacks that region, results from full-length thymosin beta-4 do not automatically transfer to TB-500 4. The new liver and brain reports are therefore not direct TB-500 evidence, but they shape what researchers watch in the fragment's family. DOI+1

What this means for TB-500 research

For TB-500 specifically, the new studies do not expand the direct evidence base. Direct fragment evidence remains the single rat Achilles tendon study 5, the Alzheimer's mouse study 7, and the eye injury model 8. The liver and brain findings tell researchers which effects are plausible in the parent peptide, but fragment-level testing is still missing 4. The brain study also keeps tumor promotion on the theoretical risk list for the peptide family 13. The TB-500 research summary should frame these new findings as parent peptide context, not as TB-500 trial data. DOI+1

What we still do not know

There is still no human randomized trial of TB-500, and no human trial of this fragment for any condition 5. The liver study has not been repeated, and the brain study has not moved into clinical testing 1. Researchers do not know whether the 17-23 fragment has the same liver protection or cancer-related actions as the full peptide 4. Those answers require direct fragment studies, and none have been published. DOI+1
